If I am preparing a refrigerated breastmilk bottle for my dear sun ahead of time, how long can he leave it out after warming it up before it goes bad?

It's cited that milk thawed from the freezer should be used within 24 hours in the fridge, or 1 hour at room temperature. My wife is exclusively pumping, and so there is a lot of milk cycling in and out of the fridge. She is always concerned about using milk taken out of the fridge (generally +/- 1 day old) within an hour or two. We often talk milk out (and let it warm up to room temperature), only to have baby fall back asleep. Is there a basis to how long fridge-temperature milk (stored at 3 degrees C for a day or two) can be out of the fridge for before it has to be used/tossed?

I read that it stays fresh for 4hours, a little more if kept in ice. Best way to be sure is to taste it, if that doesn't freak you out, it seems like a lot of mommies are iffy about this. I always throw out milk that has been "contaminated" by the bacteria in LO's mouth - ie, when he has sucked on the nipple. Or when it has stayed out for 4hours, but 2 hours, untouched, I out it back in the fridge.
