I ve never taken my dog in to the vet (please don t be disrespectful) I ve had her for four years ever since she was given to me by a old family friend (we are not in contact with them anymore) she s a mix breed (Maltese and Lhaso) and I ve been really wanting to take her to the vet more often to assure her health etc.

Yes, you can take her to the vet without any documents. Just call and make an appointment. They will probably want to do an exam as well.

Sure - when I've switched vets, I just take my hounds in. Sometimes I'm asked who I used before, but usually my hounds' information is just entered into the new vet's system. If/when they needed boosters, I'd take the old Shot Card with me for the new vet to put the new details into.
